Please download the Tattoo Consent form from here.

There is a Download button in the top right corner.

For iPhones (see images below if you need help)

• Click on the share button at the bottom of your screen after downloading

• Scroll down to select Save to Files

• In your Files app, open the Consent form file

• Fill in the consent form using the typing feature at the bottom right-hand corner of the screen, then save. Edit the image with the drawing feature next to be able to draw your signature. Save the file.

For Samsung, there should be a pop-up when you download the file. Select the file here and then fill out the form. If you do not get a pop up, the form will be in your My Files app under Downloads.

Fill out BOTH pages and sign the Tattoo Consent Form and SIGN your name on the signature lines.

Have images of the front and back of your ID available.

If you are unable to submit the consent form and photos of your ID via the Google Form, please the form and images to jordanlentztattoos@gmail.com

At your appointment, please show the email receipt to Jordan to show proof that you submitted your consent form.
